India launches website on public health

The dedicated user friendly website would greatly help in sensitising the states and cities under JNNURM and provide information relating to urban water supply and sanitation at the national level.

New Delhi: The Government of India has launched the Central Public Health and Environmental Engineering Organisation (CPHEEO) website to provide requisite information to people related to urban water supply and sanitation.

Launching the website, the Urban Development Secretary M Ramachandran said that it would greatly help in sensitising the states and cities in the context of ongoing programmes under Jawaharlal Nehru National Urban Renewal Mission (JNNURM).

The pilot project, developed by National Informatics Centre (NIC), was started in 1988-89 to put in place a Management Information Systems (MIS) on urban water supply and sanitation sector with the help of World Health Organisation.

Indian Institute of Management (IIM) of Ahmedabad has developed the MIS modules.

The Planning Commission had earmarked funds for the development of the website in successive Five Year Plans from 7th Plan to 10th Plan, the Ministry informed.

Keeping in view the development taken in IT sector and its usefulness for water supply and sanitation sector, the present CPHEEO website development task was started in May 2005.

The website consists of two parts—static part and dynamic part. The static part features Contact Us, Contact State PHEDs, Organisational Structure, Watsan related agencies and newsletters, whereas the dynamic part of website contains the information on the status of urban water supply and sanitation under "Status on Urban WATSAN".

The website provide links to the CPHEEO-approved data collection formats, which could be accessible by individual cities and towns through dedicated portals on the website to update the data periodically.

The formats have been up-linked for physical and financial status on water supply, sewerage, drainage and solid waste management, the Ministry added.

Efforts have been made for making the dynamic part of website operational, which include a proposal for launching of a centrally sponsored scheme for setting up of MIS cells at CPHEEO and state level.

The scheme, which has been envisaged and included in 11th Five Year Plan, has been recommended by Working Group on Urban Development and accepted by Steering Committee in Planning Commission.

According to the Ministry, the envisaged MIS for UWSS scheme would require Rs 8.4 crore during 11th Five Year Plan.

After the endorsement of the scheme by Planning Commission and consent of state governments to bear operating and maintenance (O&M) expenses, the MIS cells at the state level will be set up and handed over to them after initial support for first two to three years.

The new scheme, setting up of MIS cells at CPHEEO and the state level, will be put up in SFC under chairmanship of Secretary (UD) for approval.

Once the new scheme is implemented, then the much sought after information collection and dissemination on urban water supply and sanitation sector would be available at national level, which will be a planning tool for policy makers and administrators.

Health Systems an end in themselves or a means to achieving certain ends? Worldwide, there seems to be a consensus on measuring health systems in terms of improving the health status, enhancing patient satisfaction and providing financial risk protection. In 2000, the World Health Organization (WHO) further expanded the definition to include a reduction in disparities for improving health status; being mindful of the patient’s need for privacy and confidentiality and providing services promptly and with courtesy as characteristics of a responsive system; and sharing the financial burden in accordance with the ability to pay as being a fair form of health financing (World Health Report, WHO, 2000). There is, however, little consensus on what constitutes an ideal health system in universally acceptable terminology to enable better inter country comparisons. This is because, unlike any other sector, health systems are highly contextualized and influenced by various exogenous factors such as societal values, epidemiology and disease burden, availability of financial resources, technical capacity, individual preferences and the nature of demand. Technological innovation in the health sector has improved the quality of life but has also increased costs. In countries that have no social insurance and where the role of the state is limited, people spend a substantial proportion of their incomes on seeking medical treatment, and in the process, get impoverished, thus widening disparities in the health status. To contain spiraling prices and distortions created by market failures such as moral hazard, asymmetry in information, induced demand etc., countries resort to multiple policy instruments. Health systems have five aspects or knobs that interact with each other and influence its basic nature and direction: (i) financial (tax, user fees, out-of-pocket expenditure, insurance), (ii) payment systems (how providers are paid: salary, per service rendered, capitation), (iii) organizational (manner in which the delivery systems are organized/structured), (iv) legal (regulatory frameworks) and (v) social (access to health information, advertising) (Hsiao 2000).The effectiveness with which these instruments of state policy are designed and used determines the extent to which the health system is equitable, appropriate or fair. The health system in India consists of a public sector, a private sector and an informal network of providers of care operating within an unregulated environment, with no controls on what services can be provided by whom, in what manner, and at what cost, and no standardized protocols to help measure the quality of care. There are wide disparities in access, further worsened by the poor functioning of the public health system.

It is good but the general public is not getting the allocation of fund made by GOI towards the following sectors which is directly related to developement of the general masses.
1. Drinking Water
2. Sanitation
3. Education.

Initiative is good but it needs to diversify in certain areas like human rights and health, water, sanitation, environment etc.
There are certain vital issues need to be focused more. Particularly, Doctor ration, hospital beds, allocation of budget, and patient chart. Apart from it data bank is needed to explore the new branch of science to evaluate the over all performance of involved stakeholders.

An applaudable venture! Provide oportunities in the addition of a chance for the rural public to give feedback and make it people friendly; as water and sanitation are major cause of most of the public health problems attention is welcome. Please allot some area in it for education on water management and importance of trees to protect environment.

This website is the need of the hour in our country. Urbanisation has lead to a host of problems either in the form of water pollution, ground water pollution, sanitation problems, air pollution, etc which in turn had its effect on health. Hope this website would play a proactive role in minimising the effects of urbanisation on a long term and interactive basis.

"Health is wealth! If health is lost everything is lost!" are the maxims. We hear a lot about GOI and the state governments, particularly Andhra Pradesh,going all out to promote public health delivery systems, but what has been done is little or nothing. Proof of the pudding is in eating; we don't gain anything from schemes that are not translated into action.Public Health promotion schemes brook no delay. They must be pressed into action forthwith. I hope they will see the light of the day too soon!

I would like to repeat Dr. Anil Paranjape's sentiments that the title of your article is misleading. Public Health is not restricted to sanitation and water supply and encompasses issues maternal and child health, reproductive health, communicable and non-communicable diseases as well as nutrition (both under and overnutrition) that are relevant to the urban context.

Excellent initiative in the area of solid waste management. There are hardly any institutes working towards providing comprehensive solutions except a few in the private sector. This effort/initiative from the Government is the need of the hour

I find your headline extremely misleading. the website is about Public Health Engineering and provides only a very restricted amount of Public Health information as such. Water supply and Sanitation are very important areas of Public Health but what about Immunization, Antenatal Care, Maternal and Infant Mortality?

With MIS in its cradle till date, it seems finally the going seems to be in the right directions. We need to move on this and ensure its sustainability in the long run.

This is a wonderful much awaited move from GoI especially when the urban problems are increasing and the doctors and their staff from the health dept has a limited role. Public health engineers now have to take a lead.

It is a good initiative by Government of India to launch a web site on Public Health. Here the website name was not mentioned. I request you to provide the same. The main objective of our organisation is to prmote public health.
